Evidence to support the Domestic Violence 2015-11-28 incident. I made an appointment with the Sojourn Shelter to ask for advice regarding my situation. They gave me pamphlets and let me know that this was a typical cycle of abuse providing the documentation. Taking the children out of the home and participating with the Sojourn Shelter was not an option that seemed to be in their best interest. The Sojourn Shelter suggested that I document the situation and go to the hospital to report the incident. They had suggested an Order of Protection but at that stage I was still in denial and didn’t want to hurt Charles’ job or cause him humiliation.

Evidence to support the Domestic Violence 2015-11-28 incident. Billing claim from Health Alliance regarding the services provided at Memorial Medical Center on 12/3/2015. Diagnosis Description: Unspecified injury of head, initial; Stain of muscle, fascia and tendon, Headache, Nausea with Vomiting , unspecified. When I had been at Memorial Medical Center I reported this as a domestic violence which is not what the diagnosis description confirms. The additional explanation that I had to provide to the insurance company noted that this was a domestic incident where I had received direct head/neck trauma.
